Our obligations. Jack and Jill Speech Pathology is an independent practice under the ownership of business partners Paul Tyacke and Christina Tyacke. We are committed to treating your child’s personal information in accordance with privacy law.

Jack and Jill Speech Pathology needs to collect information about you or your child for the primary purpose of providing quality treatment. In order to fully assess, diagnose and treat your child, we need to collect some personal information from you. This information will also be used for the administrative purposes of running the practice such as billing you or through an insurer or funding agency. Information may also be used within the practice in the event of handover to another clinician who may continue care for your child.
Jack and Jill Speech Pathology may disclose information regarding diagnosis and treatment to your doctor or other treatment providers only with your consent. In the case of insurance claims, it may be necessary to disclose information and / or collect information that affects your treatment. Jack and Jill Speech Pathology will not disclose your information to commercial companies, however specific service or product information as deemed suitable for your speech pathology management, may be forwarded to you by us, unless you instruct Jack and Jill Speech Pathology not to forward this type of information. Your written consent will be obtained at the start of your child’s treatment in order to carry out the above activities.
Security of information collected. Your child’s information at Jack and Jill Speech Pathology is stored securely and may be held in a variety of ways. Most commonly, it may be held as an electronic record, and where this is not possible it will be held as a paper record. Some information may also be held in the form of an image including photograph, or as an audio or video recording. Only Paul and Christina have access to to your child’s information. Jack and Jill Speech Pathology takes all reasonable steps to ensure that information collected about your child is accurate, complete and up to date.
Access to your information. You may have access to your child’s information on request and if you believe that any of the information is inaccurate we may be able to amend it accordingly. If you do not provide relevant personal or health information, in part or in full, to Jack and Jill Speech Pathology, it may result in incomplete assessment. This may impact on the diagnosis and the following therapy that is provided. In the case of divorce or separation, under current law both parents, have equal rights to access information about their child under 18 years old, unless the right has legally been removed from one parent. Any court orders should be declared on first contact with us. Access to your child’s personal information may be declined in special circumstances, such as where giving access would put you or another person at risk of mental or physical harm.
